OUR HISTORY
Outreach Community Care Network was established in 1985, as a non-profit 501(c)(3) organization in response to the Human Immunodeficiency Virus (HIV) epidemic. In 2020, we expanded our continuum of care services to include primary care, substance use, psychiatric, and mental health services for all persons.
Located in Daytona Beach, we can provide community-based services to individuals living in Volusia, Putnam, Flagler, and St. Johns counties. Our services are funded through Federal, State, and local agencies and foundations, as well as private donations.

OCCN is driven by the belief that every individual served is unique, worthy of dignity, and capable of achieving personal wellness goals. OCCN cultivates a person-centered culture through strategic planning, policy development, and resource allocation that prioritizes individual autonomy, community integration, and holistic well-being.
