This assistance is available to clients who have lost their employment due to their illness, and are eligible to continue their insurance under the COBRA option. Clients who are working and eligible for insurance, but are at or below 250% of the poverty level may also be eligible to receive assistance through this program.
Housing Opportunities for People Living with AIDS (HOPWA) is a federal government program that helps pay for transitional housing, rent, mortgage, and utilities. This program is designed to assist individuals living with HIV/AIDS who are behind in their payments and are at risk of eviction and homelessness. Case management and other needed health and support services may also be available through this program.
The goal of Medicaid Waiver - Project AIDS Care is to provide an increased quality of life while reducing the need for unnecessary institutional care. Services are provided in home and community based settings. Outreach Community Care Network can enroll clients in this program who are financially and medically eligible. Service Coordinators provide a comprehensive needs assessment for each client who is eligible for services through Medicaid Waiver. An individualized Plan of Care is developed with each client, in conjunction with the client's physician, family, and significant other, as appropriate. Service Coordinators refer the client to Medicaid providers, authorize needed services based upon client choice, and monitor the delivery of services to ensure progress in meeting established goals.

The federal Ryan White Emergency CARE Act (Title II) provides funding for individuals diagnosed with HIV/AIDS who have no other financial resources available for obtaining needed services. Funding from this source can also provide a "bridge" for clients awaiting determination of eligibility for Social Security, Medicaid Waiver, or other entitlement programs. Outreach Community Care Network can refer clients for enrollment in the Ryan White service network (administered by Volusia County Health Department Ryan White Case Management team) so that (s)he can access services from enrolled Ryan White service providers. The only requirement for eligibility is proof of HIV.
