Case Management

To provide a complete continuum of care, Outreach Community Care Network offers comprehensive case management services to the residents of Volusia and Flagler Counties. Case management programs are funded by:

• Housing Opportunities for People with AIDS
• Medicaid Waiver Project AIDS Care
• Ryan White Title II CARE Act
• Volusia County Health Department

Some of the services coordinated through these programs include:

Outreach Community Care Network Case Managers develop a plan of care with the client in conjunction with their physician, family members, and significant others to arrange and authorize needed services.
