Case Management

To provide a complete continuum of care, Outreach Community Care Network offers comprehensive case management services to the residents of Volusia and Flagler Counties. Case management programs are funded by:

Housing Opportunities for People with AIDS (HOPWA)
Medicaid Waiver Project AIDS Care
Ryan White Title II CARE Act
Volusia County Health Department

Some of the services coordinated through these programs include:

Substance Abuse Treatment & Counseling	Consumable Medical Supplies
Counseling and Testing	Medical Care
Dental Care	Prevention and Patient Navigating Service
Mental Health Treatment & Counseling	Home Delivered Meals
Pharmaceuticals	Medical Transportation

Outreach Community Care Network Case Managers develop a plan of care with the client in conjunction with their physician, family members, and significant others to arrange and authorize needed services.
